Hi, I am Danush Varkur, 3rd sem medical student.This is an online elog book to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio. CASE SHEET: Chief Complaint: A 60yr old male from Valigonda came to OPD with chief complaints of Puffiness of face since 1week Bilateral edema of upper and lower extremities since 1week History of presenting illness: Patient was apparently asymptomatic 12yrs ago, then he developed Hypertension for which he is taking medication. After 8yrs, he developed diabetes mellitus for which he has been taking medication. Then, 4yrs ago, he developed high grade fever associated with dry and scaly legs associated with pain which was severe and he was unable to walk properly because of that. Then, he went to a local hospital where he was diagnosed with filariasis. He was given medication, which he followed regularly due to which his pain subsided. CASE SHEET: Chief complaints: A 26 year old female, hailing from nakerekal, agricultural worker by occupation, came with chief complaints of: - generalised weakness since 15 days back - fever 15 days back - Shortness of breath since 6 days History of present illness: The patient was apparently asymptomatic 15 days back. She then developed generalised weakness during her routine work , which subsided on rest. Then 15 days back, she developed fever, which was associated with chills and rigors at earling morning, which was subsided on medication with paracetamol and fever was recurring if medication was not taken. 10 days back, she then developed cough which was non productive cough, associated with left sided chest pain, which was not relieved on medication. 6 days back, she then develop

A 17 YEAR OLD MAKE CAME WITH FEVER AND SHORTNESS OF BREATH. Hi, I am Danush Varkur, 3rd sem medical student.This is an online elog book to discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio. CASE SHEET: Chief complaints: A 17 old male student, came with chief complaints of: - fever since 7 days - shortness of breath since 5 days - right side chest pain since 3 days increased on inspiration. History of present illness: The patient was apparently asymptomatic 7 days back. He then developed high grade fever which was associated with chills and rigors. Fever was not relieved on medication. He the developed breathlessness 5 days back, which was insidious in onset, mMRC grade 2, which was aggrevated on lying down, with no seasonal variation.
